Before Your Appointment

  • You’ll receive new patient forms to fill out prior to your visit.
  • We recommend you leave 5-10 minutes earlier than planned in case of any traffic delays.
  • We are located on the third floor. As you exit the elevator, you’ll see the Siegel, Dolt, & Plaisance door.

What to Bring

We’d love for you to bring a few things along for your first visit.


  • Photo ID (such as a valid driver’s license)
  • Insurance card (for those with dental insurance)
  • Questions, concerns, and goals for your smile

Finances & Insurance

We believe in dental care without restrictions. We treat you based on your personal needs, not your insurance plan. If you do have dental benefits, we are more than happy to submit claims on your behalf, but we do require full payment for services rendered at the time of treatment.

Adults

Good oral hygiene is vital for overall health, as gum disease is linked to serious conditions like stroke and heart disease. Regular dental visits (twice yearly) allow us to clean your teeth, detect issues early, and monitor nutrition and oral health. Here are some helpful practices you can do at home to help prevent cavities, gingivitis, gum disease, oral cancer, heart disease, and diabetes:

  • Brush at least three times a day using a fluoridated toothpaste
  • Floss daily
  • Eat a well-balanced, healthy diet
  • Avoid smoking and the use of smokeless tobacco products

Children

Research shows tooth decay is the most common chronic childhood disease, affecting 50% of first-graders and 80% of 17-year-olds, leading to 52 million missed school hours annually. Early dental care is crucial for children’s health, well-being, self-image, academic performance, and overall success. Parents play a key role in establishing good oral hygiene habits from infancy. The American Dental Hygiene Association states that good oral hygiene habits for children include:

  • Cleaning your infants’ gums after feeding with a water-soaked cloth to stimulate gum tissue and remove food.
  • Brush baby’s erupted teeth gently with a small, soft toothbrush and pea-sized fluoridated toothpaste.
  • Teach proper brushing by age 2-3 and flossing by age 7.
  • Schedule regular dental visits to check for cavities and developmental issues.
  • Encourage kids to share fears about dental visits without mentioning “pain” or “hurt.”
  • Check if your water supply at home is fluoridated – if not, discuss supplemental options with your dentist.
  • Ask your hygienist or dentist about sealants and bottle tooth decay prevention to protect teeth from sugared liquids.
